Job Summary

Under general direction of the Director of Coordinated Entry the Coordinated Entry Consultant in HRAs Office of Supportive Housing and Specialized Services will utilize clinical expertise and experience to develop implement monitor and enhance the Coordinated Entry Program. The incumbent works with social service providers and other stakeholders to match and place candidates into supportive housing in a timely manner. Knowledge and in depth understanding of available program and social services resources is key to ensure that the program is correctly identifying prioritizing and placing the most vulnerable individuals and families into the most appropriate supportive housing and rental subsidy programs.

The Human Resources Administration (HRA) is recruiting for one (1) Senior Public Health Consultant to function as a Coordinated Entry Consultant who will:

- Process the monthly placement information submitted by housing providers to ensure accuracy of move in and move out setting and reasons in the Coordinated Assessment and Placement System (CAPS). Additionally provide any technical assistance to housing providers or partners to ensure accurate information in CAPS including training system demonstrations or written direction.

- Meet with community social workers clinical directors housing providers and other community and government partners to gather information that will assist in the design and implementation of Coordinated Assessment and Placement practices procedures and systems.

- Collaborate with the Coordinated Entry Director Supportive Housing Director and relevant CAPS stakeholders to continually gather performance information on the CAPS system and provides.


Salary Range: $82950.00 - $114446.00


Hours/Schedule: Monday - Friday 9am-5pm

recommendations for enhancements/improvements. Utilizes this information to propose new policies and enhanced practices procedures and systems to reduce homelessness and ensure expeditious placement of clients into the appropriate housing programs.

- Lead regular forums and meets with participant organizations including referring agencies housing providers advocacy and other types of organizations to assist them in understanding implementing and coordinating the homeless assessment and placement process.

- Monitor and evaluate the regular collection of clinical data needed for the further development of the CAPS system the Vulnerability Index and the Housing Inventory. Provides outreach to all partners and serves as an advisor to partners needing interpretation and understanding of clinical and social service aspects to improve data quality related to the client eligibility and vulnerability assessment process the placement process and the housing inventory.

- Participate in the NYC Coalition on the Continuum of Care (CoC) and other internal and external committee meetings to keep abreast of all new COC initiatives and HUD requirements related to the CAPS system.

- Participate in projects with agency government and community social services stakeholders to enhance integration and further development of supportive housing services to homeless individuals and families with disabling conditions from shelters street and other settings.

- Monitor quality assurance activities and prepares reports in conjunction with the Director. - Participate on review committees for proposals to the agency for supportive housing and other programs.

Qualifications :

A baccalaureate degree issued after completion of a four-year course in an accredited college or university and a masters degree from an accredited school of social work plus:
1. Six (6) years of full-time paid satisfactory experience in social work including (a) two (2) years of casework experience in a health medical or psychiatric care agency adhering to acceptable standards; and (b) four (4) years in a supervisory consultative or administrative capacity or in teaching in an accredited school of social work with at least one (1) year in a health medical or psychiatric care agency; or
2. A satisfactory equivalent.
