Job Purpose


The Hospice Chaplain is a vital member of the Interdisciplinary Hospice Team providing spiritual care and support to patients and their families in accordance with their individual spiritual and religious beliefs. This role involves assessing the psycho-spiritual religious and emotional needs of patients and families and collaborating with the Hospice Team to develop and implement an appropriate spiritual care plan. The Hospice Chaplain serves a diverse patient population offering sympathetic support to individuals of all faiths and backgrounds. This is a part-time position serving the Lakes Region area of central New Hampshire.


This is a part-time or per diem position.



Primary Duties and Responsibilities

  • Provide spiritual support and counseling to hospice patients and their families respecting their unique spiritual and religious perspectives.
  • Conduct spiritual assessments to identify the psycho-spiritual religious and emotional needs of patients and families.
  • Collaborate with the Interdisciplinary Hospice Team to create and implement a comprehensive spiritual care plan.
  • Offer empathetic care to individuals dealing with life-limiting illnesses loss and grief.
  • Facilitate spiritual rituals prayers or sacraments as requested by patients or families.
  • Document spiritual care activities and patient interactions in the medical record ensuring compliance with organizational policies.
  • Participate in interdisciplinary team meetings case conferences and other relevant meetings to discuss patient care.
  • Provide crisis intervention and support in situations of trauma grief and end-of-life transitions.
  • Serve as a liaison between the patient family and community spiritual resources when appropriate.
  • Maintain professional competency in spiritual care practices and hospice care through continuing education and training.
  • Educate and support other hospice team members on spiritual care issues as needed.

Qualifications

  • Master of Divinity (.) or equivalent graduate degree from an accredited seminary or theological school.
  • Minimum of two units of Clinical Pastoral Education (CPE) from an ACPE-accredited program required.
  • Ordained commissioned or similarly recognized by a faith group to function in pastoral care ministry.
  • Minimum of one year of experience in hospice or end-of-life care; two years preferred.
  • Strong spiritual assessment skills especially in dealing with grief loss and trauma.
  • Experience in a multi-faith setting or background in social work psychology or pastoral ministry is strongly recommended.
  • Proficiency in using medical electronic health records (EMR) systems.
  • Valid drivers license in good standing and reliable vehicle insurance required.

  • This is a traveling position requiring a reliable vehicle to navigate the varied weather conditions within the Lakes Region area.

Physical Requirements

  • Prolonged periods of sitting standing and walking.
  • Visual acuity and hearing necessary to perform required duties.
