INFORMATION AND TRIAGE
Manage enquiries and referrals.
Conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
Refer clients to appropriate agencies for further intervention.
Ensure proper documentation of referrals and intakes.
Ensure reports are submitted in a timely manner.
CASEWORK AND COUNSELLING
Co-manage cases (mainly low level of complexity).
Conduct assessment planning and intervention for cases
Participate in networking and case management meetings.
Ensure proper documentation of cases.
Ensure reports on cases are submitted in a timely manner.
COMMUNITY WORK & GROUP WORK
Network with other organizations and stakeholders.
Participate in implementation and evaluation of community outreach / initiatives.
Participate in implementation and evaluation of groupwork sessions.
