POSITION SUMMARY:
The Care Management Assistant is a vital member of the Johns Hopkins All Childrens Care Network (JHACCN) care management team. They function to support the team with administrative and general office work calling patients/families from our network practices after an ER visit patient navigation through the healthcare system including reminders to schedule appointments for needed services researching sources for services such as DME home care etc. Additionally assisting with quality metric improvement activities including sharing quality metric reports with practices supporting documentation in the population health database assisting Network team with closing care gaps and other tasks to help meet the goals of our population health initiatives.
Hours:40 Hours Per Week
Location:St. Petersburg FL
KEY ACCOUNTABILITIES:
- Provides administrative support for the JHACCN care management team including making calls to locate needed services (such as DME home care etc.).
- Interacts with patients/families to assist and promote independence with care needs so that these are addressed and improved outcomes can be achieved; including reminders to schedule appointment(s).
- Research and refer patient/families to community resources to ensure disease support health education transportation to medical offices healthy meals behavioral health resources and other needed services.
- Assists with reviewing investigating and closing gaps in care for Network and Payer measures assisting team members and Network Practices when appropriate. Examples include monitoring for adherence to well-visits immunizations etc.
- Provides information to and from both internal and external facilities/resources to facilitate patient navigation in the community.
- Collaborates with the care management team in maintaining accurate and timely documentation of patient information in the population health database.
- Shares quality metric reports as needed with Network practices. Works with the quality and care management team to share best practices for improving metric performance with these practices. Quality metrics can be defined as measures that regulatory agencies and payers use to assess performance care quality and safety of contracted providers
- Guides families with completion of applications for benefit programs as needed e.g. pharmaceutical discounts community providers and government assistance.
- Works independently on initiatives that advance the technology analytics and population health requirements of the network.
- Expands knowledge and skills by leveraging resources provided by JHACH fostering optimal engagement and assessments to support and advance the networks initiatives.
- Performs under the supervision of the RN care managers on the care management team.
QUALIFICATIONS:
Education/Experience:
- A minimum of a High School diploma GED Certificate of Completion or equivalent achievement. Associates degree preferred.
- Minimum of 2 years experience in a clerical or administrative assistant role in the health care field.
At least one-year experience in a medical office.
Salary Range: Minimum 17.70/hour - Maximum 28.32/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
