What youll be doing:

  • Conducts comprehensive nutrition assessments for patients.
  • Develops and implements individualized nutrition care plans
  • Provides counselling and education and in-services to patients using trauma-informed and patient centered approach
  • Documents as per HHC and college standards including assessments interventions and outcomes in patient medical record
  • Collaborates with the clinical team to provide the best nutritional care for patients
  • Advises and develops custom menus snacks
  • Promotes healthy eating behaviors

What were looking for:

  • Bachelors degree in Human Nutrition and Dietetics from an accredited post-secondary institution
  • Valid registration with College of Dietitians of Ontario is required member of Dietitians of Canada preferred
  • 5 years previous experience in Older Adult and Eating Disorders
  • Experience with mental health and addictions populations
  • Collaborative service-oriented innovative and accountable
  • Commitment to working within Homewoods Health and Safety Standards
  • Practice in accordance within the ethical guidelines of the Dietetics profession
