What youll be doing:
- Conducts comprehensive nutrition assessments for patients.
- Develops and implements individualized nutrition care plans
- Provides counselling and education and in-services to patients using trauma-informed and patient centered approach
- Documents as per HHC and college standards including assessments interventions and outcomes in patient medical record
- Collaborates with the clinical team to provide the best nutritional care for patients
- Advises and develops custom menus snacks
- Promotes healthy eating behaviors
What were looking for:
- Bachelors degree in Human Nutrition and Dietetics from an accredited post-secondary institution
- Valid registration with College of Dietitians of Ontario is required member of Dietitians of Canada preferred
- 5 years previous experience in Older Adult and Eating Disorders
- Experience with mental health and addictions populations
- Collaborative service-oriented innovative and accountable
- Commitment to working within Homewoods Health and Safety Standards
- Practice in accordance within the ethical guidelines of the Dietetics profession
