Bariatric Patient Navigator Coordinator, Full-Time

Position Highlights:

• Job Type: Full-Time
• Location: Grand Valley Nutrition and Wellness
• Schedule: Monday - Thursday 8-5 and Friday 8-2

Responsibilities:

• Reviews the patients needs to determine whether the individual meets the programs parameters.
• Coordinates all bariatric services provided in association with the programs patients.
• Follows the patient throughout the steps of the program.
• Ensures patients receive appropriate psychological and other counseling.
• Facilitates and schedules support groups and educational events.
• Communicate with service delivery partners physicians and other health professionals to provide case coordination and develop a plan of care that facilitates the efficient use of health care resources.
• Effectively assess coach and graduate patients from care resulting in successful patient outcomes.
• Communicates with other healthcare professionals as appropriate informing them of patients progress and/or any potential concerns.
• Coordinate and document all details of patients progress throughout and after program participation.
• Facilitating/leading the hospital bariatric program Multidisciplinary Task Force ensuring coordinated comprehensive program that includes nurses dietitians and physician extenders.
• Develop and maintain bariatric partnerships with outlying hospitals to assist patients to get surgery at Community Hospital and have pre and post-operative care and support done in home community as needed.

Requirements:

• High School diploma or equivalent; Associates Degree or Bachelors Degree in healthcare preferred.
• Previous proven experience and expertise in Surgery scheduling insurance authorizations and pre-surgical testing with a minimum five (5) years in healthcare services preferred OR equivalent combination of education and experience.

Compensation:

• $21.00 - $24.15 per hour depending on education and experience.
• Discretionary bonuses relocation expenses merit increase market adjustments recognition bonuses and other forms of discretionary compensation may be available.
