Position Details:
We are seeking a friendly and compassionate PCP Nurse Practitioner to join our team. As a PCP Nurse Practitioner you will play a crucial role in providing comprehensive care to patients in long-term care facilities. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.
Responsibilities:
- Provide comprehensive healthcare services and coordinate patient care
- Work closely with a team of healthcare professionals
- Assess diagnose and treat residents physical and mental well-being
- Conduct thorough patient assessments including medical history reviews and physical examinations
- Order and interpret diagnostic tests
- Develop accurate diagnoses and treatment plans
- Prescribe medications and administer treatments
- Provide counseling to residents and their families
- Collaborate with the interdisciplinary team for seamless coordination of care
- Manage chronic conditions and monitor progress
- Make appropriate adjustments to care plans
- Commitment to person-centered care
- Create a safe and nurturing environment for elderly residents in the nursing home.
- Documenting billable patient care services into an electronic health record system using accurate ICD-10 & CPT CODES
- Possibly rotating to more than 1 facility near Millersburg PA
Requirements:
- Nurse Practitioner License
- DEA License
- 6 months experience in long-term care
