SUMMARY
Provides psychosocial assessment care planning and intervention for a designated caseload of patients and families. Coordinates referrals and continuity of care collaborating with the multidisciplinary team to optimize patient outcomes. Supports departmental goals and provides coverage during absences vacancies and on-call periods.
COMPETENCIES
A. Education
B. Knowledge
Understanding of biopsychosocial and cultural factors in clinical practice.
Familiarity with social work systems procedures and community resources.
Awareness of the current health care environment and ability to synthesize data for patient care decisions.
C. Skills
Strong oral and written communication skills.
Proficiency in clinical social work within mental health or health care settings.
Analytical ability to assess needs develop treatment plans and recommend improvements in care.
D. Licensure/Certification
E. Experience
Salary Range: Minimum 31.92/hour - Maximum 52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
