Your role and responsibilities
- In this role you will be supporting the assessment team with casediscussions and supporting with complex cases including mental capacity assessments. You might complete home visits with assessment staff to adults in their own homes
- You will be supporting the practice development of the team through sharing your expertise and keeping up the date with the latest processes and information
- Day to day you will be supporting with working alongside other colleagues in Home First to support demand on the service and allocation of work including holding a small number of cases when capacity demands this
- You will utilise your recording skills to support the team to create excellent holistic assessments
You will need
- For this role you will need to be a qualified and registered Social Worker.
- Our ideal candidate is enthusiastic about excellent social work practice and is able to encourage others to feel the same way.
- The person most suitable for the role will be passionate about supporting the people of Suffolk to remain as independent as possible athome and would ideally have some knowledge of reablement support.

The team
- Home First is the in-house Reablement Service in Suffolk County Council. Our Aim is to support people who need ashort-term period of reablement and assessment to lead their lives at home and as independently as possible. To do this our teams need to think creatively and fast!
- Our teams work closely with the local INT and area teams alongside health colleagues and local home care providers. These good relationships are vital for supporting a holistic assessment of the people we look after and were very proud of the strong team ethic we have created.
- We practice hybrid working within the team but we have designated office days to promote in-person communication to foster excellent working relationships.
