This position provides clients with case management in order to help them achieve their goals while ensuring a safe space while engaging with HYS staff. The case manager helps clients learn life skills build positive relationships and navigate public and social services available in the community.
The scheduled shifts are 4 days per week working 8:00 AM - 6:30 PM.
MINIMUM QUALIFICATIONS
The requirements listed below are representative of the knowledge skill and/or ability required.
EDUCATION and/or EXPERIENCE
- High School Diploma or GED required.
- Four or more years of work or volunteering experience in fast-paced customer service healthcare education social or community services or related public-facing sectors; or equivalent combination of education experience and training may be accepted.
- College course work in social sciences or related human services preferred but not required.
- Preference will be given to candidates with work or volunteer experience working with special needs population and/or experience working with youth or high-risk youth.
CERTIFICATES LICENSES REGISTRATIONS
- Driver qualifications are required for this position. Valid driver license insured vehicle available for use on the job and a satisfactory drive record for the previous three years are required.
- Candidate must pass a comprehensive MWVCAA background screening and an Oregon Department of Human Services criminal background check prior to employment.
- Candidate must pass pre-employment and random drug/alcohol screenings.
- CPR/First Aid Food Handlers card Mandatory Reporting Privacy Laws Trauma Informed Care Suicide Intervention Behavior Management Interventions and Recognizing and Reporting Child Abuse & Neglect trainings and/or certifications required within the first 30 days of employment.
KNOWLEDGE SKILLS AND ABILITIES
- Knowledge of/or experience with: the principles of coordinated assessment working with unsheltered or vulnerable populations as well as best practices in homeless service delivery is preferred.

- Must possess excellent planning organization and time management skills.
- Ability to effectively communicate both orally and in written form.

- Works directly with clients to ensure access to housing assessments and resources.
- Assesses client eligibility and aids in gathering documentation as required for intake enrollment and shelter placement.
- Provides direct service to participants including case management and housing placement support to assist participants in becoming self-sufficient and overcoming barriers to accessing permanent housing.
- Develops Action Plans that identify participant needs barriers and strengths.
- Establishes and maintains an active working relationship with community partners.
- Provides support for client-tracking service delivery follow-up reporting and maintaining strict confidentiality.
- Maintains full compliance with all federal state and grant program and funding requirements setting high standards for data quality and compliance performance remaining in compliance with the regulations of all funding streams.
- Works cooperatively with other Arches Project programming to ensure quality and timely client service delivery.
- Complies with agency Quarterly and Annual reports.
- Complies with MWVCAA safety and personnel policies including punctuality and attendance.
- Performs cleaning janitorial and laundry tasks in support of day-to-day sheltering operations.
- Supports nutrition staff as needed by completing various kitchen tasks including cooking meals.
- Provides client transportation in Agency vehicles to and from school to and from appointments and to other assigned outings.
- Participates in Community Outreach to local organizations camps and groups as directed by supervisor.
- Maintains appropriate boundaries with clients and coworkers at all times.
- Attends and engage required annual trainings.
- Assists with filling prescriptions for clients with case worker approval.
- Facilitates dispensing of medications to clients following proper protocols.
