Purpose: Oversees the nutritional well-being of patients and the community. The successful candidate will have a proven track record of conducting nutrition assessments identifying patients at risk and creating diet plans.
Job Duties:
- Ensures patients nutrition plan selections are in compliance with prescribed diet order nutritional adequacy age cultural background and religious practice.
- Writes and modifies nutrition plans as needed according to patient diet prescription using established resources/guidelines.
- Provides cooking demonstrations for community members and patients
- Updates monitors and follows-up on all pertinent patient information like diet order changes food allergies food preferences and requests.
- Prints and processes meal plans and food tally report.
- Serves as a dietary resource for nursing and other staff.
- Provides general information on diet utilizing approved materials and resources.
- Incorporates physical activity plans as part of the overall diet and wellness outcomes
- Works in conjunction with a multi-disciplinary team
- Attends meetings coalitions committees as assigned
- Performs other duties as assigned

- Listening analyzing information and focusing on quality
Education and Experience Requirements:
- Bachelors degree in food nutrition food service management or related field
- NYS License
- Accredited Dietetic Internship (DI) by the American Dietetic Association (ADA) Preferred
- Food Handlers permit or ServeSafe certification as per state requirements Preferred
