Community Support Worker

Role Overview
The Community Support Worker provides direct in-person non-clinical support to adults with disabilities living in continuing care facilities. The role focuses on helping individuals participate in meaningful community activities reduce social isolation and achieve personal goals identified through Individualized Support Plans (ISPs).
Key Responsibilities
Intake & Assessment
- Participate in intake and eligibility confirmation
- Conduct needs-based assessments under program guidelines
- Collaborate with clients to identify interests goals and barriers
Individualized Support Planning (ISP)
- Develop Individualized Support Plans in collaboration with clients
- Review and update ISPs regularly based on progress and changing needs
- Ensure client goals are respected and prioritized
Direct Client Support
- Provide in-person accompaniment to community activities
- Assist clients with:
- Transportation
- Social and recreational outings
- Appointments and community engagement
- Facilitate participation in activities aligned with ISP goals
- Provide in-facility programming when community outings are not feasible
Documentation & Reporting
- Maintain accurate records of services delivered
- Track client participation and outcomes
- Submit documentation to Program Manager in a timely manner
Collaboration & Safety
- Communicate effectively with care facility staff
- Follow safety confidentiality and professional conduct standards
- Identify and report risks or concerns promptly
Required Qualifications
- Diploma in Human Services Social Work Community Support Disability Studies or related field
- Minimum 2 years experience supporting adults with disabilities
- Experience with needs-based assessments and person-centred planning
- Strong interpersonal and communication skills
- Valid drivers license and access to reliable transportation
