SUMMARY
Under general supervision the Case Coordinator determines eligibility for the Countys Indigent Health Care Program in line with Texas Health and Human Services guidelines. This role involves maintaining client records managing provider relationships and referring ;individuals to appropriate social services. The coordinator communicates with clients and providers about eligibility benefits and policy updates and holds office hours in various County locations. Additional responsibilities include attending state and regional training researching alternative funding sources coordinating with other agencies monitoring Social Security status for Medicaid reimbursement and ensuring compliance with HIPAA regulations.

EDUCATION AND/OR EXPERIENCE:
A high school diploma or GED equivalent is required. An Associates or bachelors degree in Business Health Care Administration or a related field is preferred but not mandatory. Candidates must have at least five (5) years of experience in health care social services or case management along with a minimum of two (2) years of office experience. Proficiency in basic computer applications including Microsoft Office is required. Bilingual in Spanish/English is required Within one (1) year of hire the employee must complete both Basic and Advanced Training through the Texas Department of State Health Services and be certified in CPR. Must have a valid Texas drivers license to operate county vehicles.
LANGUAGE SKILLS
Bilingual English/Spanish required for verbal and written communication with clients.
Ability to read and interpret documents such as safety rules operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts interest commissions proportions percentages area circumference and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written oral diagram or schedule form.
