Job Summary
- Provides direct patient services that are physician ordered.
- Provides hands-on personal care and support services under the direction and supervision of a RN.
- Provides assistance to patients with ambulation and safe transfers per agency policies.
- Reports and condition changes to Case Manager and documents any services provided and changes noted in patients condition.
- Participates in care coordination meetings per company policy.
- Must be able to lift up to 50 lbs and assist with transfers of patients.
- Must be able to drive around Metroplex area to patients residence to provide care.
- Must respect patients home and environment.
- Must keep up required continuing education requirements for a CNA.
Qualifications :
- Current CNA license
- Current Drivers License
- Must have 1 year experience in Home Health nursing home or hospital setting.
- Must be computer literate and be able to document electronically.
- Must have current CPR certificate
- Must be able to pass State required onboarding testing.
- Must have reliable transportation and current auto insurance
