This is a unique opportunity to be part of a tight-knit collaborative team you will assist palliative patients with personal care and activities of daily living. You will support patients families and caregivers enhancing quality of life comfort dignity and security in patients end of life journey. All direct patient care activities will be performed under the direction of an Integrated Care Program Supervisor who is a nurse.
- Working alongside the interdisciplinary palliative care team you will provide specialized care as specified in the care plan according to protocols within scope of practice and in accordance with Spectrum Health Cares policies and procedures.
- Provide companionship and comfort creating a calm and respectful environment tailored to the clients needs preferences and goals of care.
- Assist person(s) with activities of daily living such as bathing toileting hygiene skin care light meal preparation feeding and transfers.
- Perform light housekeeping duties such as decluttering and cleaning
- Listen observe and communicate any concerns/changes in the patients physical and/or emotional condition to the Integrated Care Program Supervisor.
- Travel within assigned region between patients residences or facilities.
Qualifications :
- Have one of the following qualifications (required):
- Valid Personal Support Worker Certificate from a Ministry of Training Colleges and Universities or the Ministry of Education approved program
- RN/RPN Student with minimum successful completion of year-one of the program
- Internationally licensed Nurse Midwife or Physician
- Minimum 1 year experience with palliative patients
- Successful completion of one of the following an asset:
- Fundamentals of Hospice Palliative Care; or
- Core Concepts of Fundamentals for PSWs; and
- Additional and on-going relevant palliative education
- Current CPR and First Aid Certificate
- Current Police check suitable for working with the vulnerable sector
- Must possess the physical ability to perform the day-to-day duties described in the duties and responsibilities
- G2/G License with access to a vehicle
