As a Registered Nurse in LTC youll deliver essential person-centered care for residents across the long-term care continuum. Your responsibilities include thorough nursing assessments medication administration and reconciliation wound and IV care ongoing vital sign monitoring and rapid responses to clinical changes. Youll develop and regularly update individualized care plans in collaboration with residents families and the interdisciplinary team ensuring goals are clear achievable and aligned with residents preferences. Youll lead with clinical judgment mentor nursing assistants coordinate with physicians and specialists and participate in care conferences and discharge planning to optimize outcomes.
