We provide a team-based model of care including physicians RNs LPNs Home Health Aides physical/occupational therapists social workers transportation a day center team and more! We are privileged to serve patients who are 55 years of age and older helping them delay/avoid nursing home placement and remain living in the community as long as possible. Currently most of our patients reside at the Sisters of St. Joseph; we also care for patients in the surrounding community.
This is a truly unique program our region is fortunate to have access to with only 130 PACE programs across the country!
Occupational Therapist Part-Time Eddy Senior Care-Latham New York
Eddy Senior Care PACE is seeking a dedicated Occupational Therapist to provide evaluation treatment planning and therapeutic interventions for participants. This is more than a jobits an opportunity to transform lives while working in a supportive mission-driven environment.

What you will do:
- This role focuses on improving functional independence safety and quality of life through individualized care plans.
- Collaborate with interdisciplinary team members to ensure integrated care planning.
- Conduct comprehensive evaluations and develop individualized treatment plans.
- Implement therapy programs focused on functional independence and occupation-based activities.
- Educate participants and caregivers on adaptive equipment home exercise programs and safety strategies.
- Prepare timely and accurate documentation in compliance with regulatory and organizational standards.
- Participate in team meetings quality improvement initiatives and in-service education.
- Coordinate ordering and management of durable medical equipment (DME) as needed.
What you will need:
- NYS Occupational Therapist Licensure. NBCOT certification a plus.
- Minimum 2 years experience as an Occupational Therapist with at least 1 year working with older adults. Experience in PACE and home health preferred
- Strong clinical judgment communication and organizational skills;
- Proficiency with EMR systems and Microsoft Office.
- Valid Drivers License and Reliable Vehicle. Good driving record

Pay Range: $35.85-$45.96 Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
