From primary care doctors to pediatricians and surgicalspecialists we have physicians to support patients whole health at every age and every stage of aligned model and practice management experience provides the opportunity for physicians tofocus on the clinical aspects of :Comfort with using clinical expertise in an independent with DSM-5 diagnostic criteria and treatment planning based on written and verbal communication ability to establish rapport quickly with a wide range of people. Provenability to work within and coordinate a care team comprised of internal teammembers and community-based membersConduct initial intake visit including detailed behavioral health and substance usehistory biopsychosocial history and assess suicide for providing mental health treatment to patients and assuring all patientsreceive whole person or related fieldMinimum 2 years direct clinical experience delivering behavioral health care in arelevant formal training and experience in brief structured counseling techniques ( behavioral therapy (CBT) or (PST)LCSW - State of Colorado Licensed Clinical Social Worker or LPC (LicensedProfessional Counselor)Qualifications - ExternalEXPERIENCE PREFERRED:Experience successfully working on multi-disciplinary working with medically ill patients in a primary care working with all age ranges of benefits package including:Medical dental and vision coverage403b/457b contribution and matchCommitted and collaborative leadership teamEstablished and successful programCME allowance
