Key Responsibilities

  • Provide leadership and management for early identification assessment and support for children with SEND and their families.
  • Develop referral pathways and work closely with multi-disciplinary teams.
  • Provide advanced clinical advice and hold a small caseload for families needing extra support.
  • Champion partnership working communication and information sharing.
  • Undertake audits research and present findings to drive quality improvement.
  • Uphold the highest standards of clinical governance confidentiality and safeguarding.

What Youll need to succeed:

  • Specialist Community Public Health Nurse qualification Health Visiting or School Nursing.
  • Experience working with SEND families and multi-disciplinary teams.
  • Leadership skills effective communication and a passion for service improvement.
  • Commitment to safeguarding equality and outstanding customer experience.

The Service & Team:

The role will be part of the Clinical Practice Team within Best Start Community Health and Wellbeing service working together to promote high-quality clinical standards and improve health outcomes for children families and communities.

The Role:

We are seeking two experienced professionals to join our Clinical Practice Team. These roles will cover the East and West of the county. There are a total of 60 hours available across both positions offered at Band 7 under Agenda for Change terms and conditions.

This is a public/customer-facing role where the statutory English language requirement for public sector workers applies.

This position will be subject to an enhanced criminal record disclosure check.

Working Pattern:

Hours are as stated and the working pattern can be discussed at interview. Your base will be New County Hall Truro with opportunities of hybrid working from other bases or home.
