Position Overview
We are seeking an experienced Home Health Occupational Therapist to deliver high-quality patient-centered care in the home setting. This role offers autonomy flexibility and collaboration with an interdisciplinary care team.
Responsibilities
Perform comprehensive OT evaluations in patients homes
Develop and implement individualized treatment plans
Provide skilled interventions to improve function mobility and safety
Educate patients and caregivers on exercises safety and injury prevention
Monitor progress and adjust care plans as needed
Document all visits accurately and timely in the EMR
Collaborate with physicians nurses and care team members
Maintain compliance with Medicare HIPAA and agency standards
Qualifications
Graduate of an accredited OT program
Active Pennsylvania OT license
Minimum 3 years of clinical experience (home health preferred)
Strong assessment clinical reasoning and documentation skills
EMR experience preferred (Kinnser/WellSky or Homecare Homebase)
Reliable transportation and valid drivers license
COTA supervision experience preferred
