Description

Position at Sharon Hospital

Summery: Responsible for providing social service and discharge planning functions for the inpatient behavioral health unit. Completes psychosocial assessments contributes to the integrated treatment plan runs therapeutic groups coordinates discharge plans. Maintains performance improvement activities within the department and participates in PI activities. Population served based on scope of services in the department.
Essential Responsibilities:
1. Responsible to perform psychosocial assessments within 72 hours of admission.
2. Identifies and addresses psychosocial needs of patient/ family and provides support counseling and crisis intervention.
3. Ability to assist physicians and communicate social service post-hospital care and discharge planning needs of the patient.
4. Leads insight-oriented psychoeducational process or skills groups.
5. Runs family meetings with both the physician and patient as needed.
6. Provides support with advance directives and assists patient/family with the grieving process and makes referrals to Pastoral Care as appropriate.
7. Accurately determines and communicates the type of assistance setting or resources necessary for the patient/family and provides appropriate resources.
8. Resolves personnel concerns at the departmental level utilizing the appropriate processes as required.
9. Maintains safety and regulatory requirements including all state federal and Joint Commission regulations.
10. Documentation meets current standards and policies.
11. Maintains current knowledge of resources available within the community maintains supply of resource materials to be distributed to patients when needed.
12. Acts on performance improvement issues identified by behavioral health leadership team.
13. Maintain and Model REACH Values (Respect Excellence Accountability Compassion Honor).
14. Demonstrates regular reliable and predictable attendance.
15. Performs other duties as required.
Requirements:
  • Master Social Worker (MSW).
  • Licensed Master Social Worker (LMSW).
  • Minimum of three (3) years experience in all aspects of social service and discharge planning.
  • Connecticut Licensed Clinical Social Worker (LCSW) or license eligible
PREFER:
  • Two (2) years of social work experience.
  • Bilingual English/Spanish
Minimum Knowledge Skills and Abilities Requirements:
Knowledge of Microsoft Office including Word Excel PowerPoint and Outlook.
Basic computer knowledge.
Excellent customer service skills.
Ability to multitask make decisions and communicate effectively.
Able to communicate effectively in English both verbally and in writing.
