Position Title: Physical Therapist Home Care
Location: Wall Township NJ (07753)
Job Type: Contract Local or Travel
Shift: 5x8 Day Shift
Duration: 13-week assignment
Pay Rate: $58 $62/hr

Job Summary:
Navitas Healthcare LLC is seeking a skilled Home Care Physical Therapist (PT) to provide in-home evaluations and treatments to improve patients mobility strength balance and functional independence. The PT will develop personalized treatment plans educate patients and caregivers and collaborate with the interdisciplinary home care team to support safe recovery and prevent rehospitalization.

Key Responsibilities:

Conduct comprehensive physical therapy evaluations in patients homes.
Develop and implement individualized treatment plans based on physician orders and patient goals.
Provide therapeutic exercises gait and balance training and functional mobility interventions.
Assess home safety and recommend adaptive equipment or modifications as needed.
Educate patients and caregivers on exercises fall prevention and safety strategies.
Monitor patient progress and adjust treatment plans accordingly.
Document evaluations treatments and outcomes accurately in EMR.
Communicate effectively with physicians nurses case managers and other home care team members.
Ensure compliance with home care regulations infection control and safety standards.
Maintain timely visit scheduling and meet productivity expectations.
Perform other related physical therapy duties as assigned.

Qualifications:

Required Skills & Experience:

Minimum 2 years of Physical Therapist experience
Previous home care experience required
Strong assessment clinical reasoning and patient education skills
Ability to work independently in a home-based setting
EMR documentation experience

Preferred Experience:

Experience with geriatric and chronic disease populations
Experience in post-acute or rehabilitation care

Required Certifications & Licensure:

Active New Jersey Physical Therapist License
BLS (AHA)
Valid drivers license and reliable transportation
