Local Pay Rate: $42-$45 hourly
Travel Pay Rate: $50-$53 hourly

Position Overview:

Navitas Healthcare LLC is seeking LPN - LTC/SNFfor an exciting Travel or Local job inFalmouth MA.

Qualifications:

• Graduate of an accredited LPN program; current active MA LPN license required.

• BLS / CPR and AED certification required.

• Strong knowledge of nursing/medical practices long-term care standards and regulatory compliance.

• Leadership and supervisory abilities with a teamwork-focused approach.

• Excellent communication documentation and organizational skills.

• Ability to lift a minimum of 50 lbs and meet all physical/sensory demands of the position.

Key Responsibilities:

• Supervise and direct daily functions of nursing assistants in alignment with facility guidelines.

• Assist in shift planning and coordination of nursing services and activities.

• Complete and maintain resident documentation for admissions transfers discharges treatments and care plans.

• Accurately transcribe and carry out physician orders; administer medications and treatments per protocol.

• Monitor and document resident responses to care reporting changes to physicians and supervisors.

• Perform skilled nursing services (IV/IM meds catheterization tube feedings wound care suctioning colostomy care specimen collection etc.).

• Conduct vital signs monitoring and routine assessments.

• Ensure adequate medication supply and equipment inventory.

• Provide patient/family health education and orientation for new residents and families.

• Maintain confidentiality of all resident information in compliance with HIPAA and residents rights.
