In this role your primary responsibilities will center on developing implementing and updating individualized treatment plans in collaboration with supervising therapists and the broader rehabilitation team. Youll conduct patient evaluations measure outcomes and adjust interventions to maximize functional gains; document progress with precision for compliance and reimbursement; educate patients and families on home programs and safety strategies; and participate in team conferences to align goals with overall discharge plans. Youll facilitate therapeutic activities that foster strength coordination cognition and endurance while applying evidence-based modalities to optimize participation in daily tasks and meaningful occupations. Youll also contribute to program development by identifying opportunities for process improvements sharing best practices with colleagues and supporting new staff through onboarding and coaching. This assignment provides 38 guaranteed hours per week with a competitive weekly pay range of 1298 to 1331 dollars and a structured schedule designed to support consistent patient contact and meaningful outcomes. Start date is 01/26/2026 and the duration is Weeks with extension opportunities depending on performance site needs and patient volume.
