Key Responsibilities:
1. Client Services
- Provide direct case management using the Critical Time Intervention (CTI) model.
- Conduct in-reach within hospitals and correctional facilities to engage eligible individuals during the discharge planning process.
- Assess clients for housing instability and determine appropriate diversion or prevention interventions.
- Develop individualized housing and service plans that address short- and long-term stability.
- Connect clients to housing resources including but not limited to mediation with landlords family reunification emergency assistance and community-based supports.
- Assist with securing documentation (ID income verification housing history etc.) necessary for housing placement.
- Facilitate warm handoffs to community providers when appropriate to ensure continuity of care.
2. System Coordination & Partnership
- Build and maintain collaborative relationships with social workers and reentry staff at healthcare systems and correctional institutions.
- Coordinate closely with emergency shelter staff outreach specialists behavioral health services and housing providers.
- Participate in case conferencing and coordination meetings with community stakeholders.
3. Documentation & Compliance
- Maintain accurate timely and confidential documentation of all client interactions and service plans.
- Input all client data into the HMIS (Homeless Management Information System) in compliance with grant requirements.
- Track program outcomes and complete reports required by funders and supervisors.
4. Professional Development
- Participate in ongoing training in CTI trauma-informed care motivational interviewing and other relevant models.
- Stay informed about local housing resources policies and service system changes.

Requirements
Qualifications:
REQUIRED:
- Bachelors degree in Social Work Psychology Human Services or related field (equivalent experience may be considered).
- Minimum 2 years of experience working with individuals experiencing homelessness behavioral health needs or involved in reentry systems.
- Knowledge of housing-first and trauma-informed care principles.
- Strong understanding of institutional discharge systems and housing instability indicators.
- Proficient in case documentation goal planning and client engagement techniques.
- Valid drivers license and access to a reliable vehicle; ability to travel locally to sites and partner locations
PREFERRED:
- Familiarity with the Critical Time Intervention (CTI) model.
- Experience working with hospitals mental health facilities or corrections.
- HMIS or comparable data entry system experience.
Hours and Schedule:
- This is a full-time benefit eligible role
- 40 hours per week
- Monday - Friday
- Flexible schedule
Physical Requirements:
- Must be able to walk up and down stairs.
- Must be able to lift up to 30 pounds at times.
- Prolonged periods of sitting standing and walking around.
