Experience Requirements
- Minimum 2 years of recent hospital (inpatient) pharmacist experience.
Education Requirements
- Bachelors Degree in Pharmacy or PharmD from a U.S.-accredited School of Pharmacy (ACPE).
Licensure & Certifications
- Active New York State Pharmacist License required.
- BLS (AHA) required.
Position Overview
The Pharmacist I provides inpatient pharmaceutical care by preparing and dispensing medications offering pharmacological guidance to the multidisciplinary healthcare team and monitoring patient drug therapies. The role requires strong clinical judgment effective communication skills and the ability to work efficiently in a fast-paced hospital environment.
Key Responsibilities
Prepare and dispense medications accurately and in a timely manner. Provide pharmacological information to physicians nurses and healthcare staff. Monitor and evaluate patient drug therapies for safety and effectiveness. Resolve questions related to formulary and non-formulary medications following policies and procedures. Provide dosing and therapeutic recommendations based on patient age pharmacokinetics and current literature. Perform patient education including proper dosing timing side effects and potential drug interactions. Document pharmaceutical care activities per department requirements. Utilize infusion-related systems and electronic health record (EHR) systems effectively.
Skills & Competencies
Strong clinical expertise and communication skills. Sound professional judgment. Proficiency with infusion systems and EHR platforms. Ability to manage operations in a fast-paced environment.
