Position Summary

Navitas Healthcare LLC is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide comprehensive social work services to patients and their families. This role plays a critical part in care coordination psychosocial assessment and discharge planning while working collaboratively with interdisciplinary healthcare teams to support optimal patient outcomes.

Key Responsibilities

Conduct psychosocial assessments and develop individualized care plans
Provide counseling advocacy and crisis intervention services
Coordinate community resources and facilitate referrals as needed
Assist patients and families in navigating healthcare systems and social services
Ensure accurate timely and compliant clinical documentation
Collaborate closely with physicians nursing staff and interdisciplinary team members

Required Qualifications

Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW)

Education

Masters Degree in Social Work (MSW) from an accredited institution

Licensure & Certification

Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED)
Primary Source Verification of licensure required
