Responsibilities
What you will do: Responsibilities listed include but not limited to:
- Work with insurance and payers to advocate for the appropriate rehabilitation and medical care. Conduct comprehensive assessments of patients medical social and psychological needs
- Collaborate with the treatment team to develop individualized care plans.
- Coordinate and monitor the delivery of services including therapy medical care and support services
- Advocate for patients rights and ensure their needs are met throughout their rehabilitation journey
- Provide education and support to patients and their families
Qualifications
What you will need:
- Bachelors or Masters degree in a related field preferred (e.g. Social Work Nursing Psychology)
- Clinical case management experience preferably in a rehabilitation or healthcare setting
- Strong interpersonal and communication skills
- Knowledge of brain injuries and rehabilitation preferred
