Support at Home Clinical Community Registered Nurse
MondayFriday Permanent Part-Time Role choice of 5 days at 9:00am to 3:00pm or 3/4 days at 8:30am to 4:30pm

Duties and responsibilities include:
Monitoring and following up Critical Incidents and Hazards undertaking Clinical Assessments within the community completing Health Care Plans supporting with clinical input into Care Plans and capturing quality in home Assessments. We are participant centric and evidenced based. We focus on wellness enablement and reablement strategies and look for participant centric goals and outcomes. We liaise with community stakeholders ACAT My Aged Care PRODA and other regulatory bodies GPs specialists and allied health specialists to ensure the best outcomes possible. There is also daily administration and compliance requirements always something to do where accuracy and attention to detail is important to achieve 100% in audits as well as occasional hands on clinical requirements. Experience working in compliance and auditing would be an advantage.
About you:
Essential requirements include strong computer and administrative skills (CRM experience essential) excellent multitasking prioritisation and problem-solving abilities to excel in a fast paced working environment. High-level customer service and communication skills. Ability to work independently and as part of a team. Experience working to KPIs billing for time. Essential:- APHRA experience in community aged care and awareness of new Support at Home program.
What you will need:
Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
Evidence of continued professional development
NDIS Worker Screening Clearance / Criminal Record Check
Current CPR / First Aid Certificate
Current Australian Drivers Licence
Reliable registered and insured vehicle (comprehensive insurance is preferred)
Previous experience in the community care or health/aged care sector (preferred)
