The Position
The Physical Therapist (PT) works as an integral member of an interdisciplinary healthcare team providing high-quality therapy services to older adults in Lowell MA between the Adult Day Health Center and in their homes.

Responsibilities

  • Monday- Friday Schedule: 8am-4pm
  • No Evenings or Weekends
  • Average of 6 Patients a Day
  • Provides clinical supervision and guidance to Physical Therapist Assistants (PTAs) and Rehabilitation Aides ensuring appropriate delegation of care adherence to therapy standards and timely resolution of staff concerns or performance issues.
  • Works collaboratively with the Rehab Director to develop implement and communicate rehabilitation policies procedures and departmental objectives that support high-quality physical therapy services.
  • Delivers direct physical therapy services including patient evaluations screenings individualized plan-of-care development and therapeutic interventions in both the Adult Day Health (ADH) setting and participants homes.
  • Coordinates physical therapy services across the interdisciplinary care team actively participating in enrollment reviews care planning meetings discharge planning staff meetings and family conferences as needed.
  • Conducts home safety assessments and provides education instruction and counseling to participants and their families regarding physical therapy treatment mobility and safety recommendations.
  • Maintains accurate and timely clinical documentation supports quality improvement initiatives complies with all regulatory safety and infection-control requirements (including respirator fit testing) and upholds confidentiality fiscal responsibility and organizational policies.

Requirements

  • Current Massachusetts Physical Therapist license
  • 1 year of clinical or healthcare experience working with older adults
  • Proficiency with Microsoft Office applications (Word Excel PowerPoint Access)
  • Ability to pass a fit test (position may require use of a sealed face mask)
  • COVID-19 vaccination preferred
