Job Summary
The Registered Nurse (RN) Hospice provides compassionate patient-centered nursing care to individuals and families facing life-limiting illness. This role focuses on symptom management care coordination and emotional support while delivering high-quality hospice services in the home and community setting.
Key Responsibilities
Perform comprehensive nursing assessments and develop individualized hospice care plans.
Provide skilled nursing care symptom management and pain control in accordance with hospice standards.
Coordinate care with physicians interdisciplinary hospice team members patients and families.
Educate patients and caregivers on disease progression medications comfort measures and end-of-life care.
Administer medications treatments and IV therapy as ordered (if certified).
Accurately document assessments interventions and patient responses in the electronic medical record (EMR).
Participate in interdisciplinary team meetings and care conferences.
Provide emotional support and guidance to patients and families throughout the hospice journey.
Travel extensively within the service area to deliver home-based hospice care.
Participate in on-call weekend and holiday coverage as required.
Required Qualifications
Active Registered Nurse (RN) license in the state of Massachusetts.
Minimum 2 years of Hospice RN Case Management experience.
Valid drivers license with the ability to travel throughout the service area.
Preferred Qualifications
Hospice home care experience.
IV certification.
Strong computer skills with EMR documentation experience.
Bachelor of Science in Nursing (BSN) preferred.
