Job description
The Outpatient Physical Therapist provides evaluation and treatment for a mixed caseload of orthopedic neurologic and cardiopulmonary patients in a clinic-based setting supporting functional recovery and safe community living. This rapid response role requires a therapist who can quickly integrate into an existing team manage a full daily caseload and document efficiently in NextGen EHR.
Key responsibilities

Perform comprehensive physical therapy evaluations including history-taking movement analysis special tests and outcome measures to establish functional diagnoses and patient-centered goals.

Develop and implement individualized treatment plans for orthopedic neuro and cardiopulmonary conditions using therapeutic exercise manual therapy balance and gait training and functional activities.

Manage a caseload of approximately 10 14 patients per day organizing the schedule to optimize patient flow productivity and timely follow-up care.

Conduct fall risk assessments recommend safety strategies and integrate fall-prevention interventions into treatment plans for at-risk patients.

Provide ongoing patient and family education regarding home exercise programs body mechanics self-management strategies and condition-specific precautions.

Coordinate basic case management tasks related to therapy needs including communicating progress and recommendations to referring providers and care team members.

Document evaluations daily treatment notes progress reports and discharge summaries accurately and efficiently in NextGen EHR ensuring compliance with regulatory and payer requirements.

Monitor clinical outcomes and adjust plans of care based on patient response evidence-based practice and established goals.

Adhere to clinic policies professional standards and infection control and safety protocols while maintaining a professional appearance consistent with a business casual dress code.
