Job Summary

SAGE is the countrys largest and oldest organization dedicated to improving the lives of lesbian gay bisexual and transgender (LGBTQ) older people. Founded in 1978 and headquartered in New York City SAGE is a national organization that offers supportive services and consumer resources to LGBTQ older people and their caregivers advocates for public policy changes that address the needs of LGBTQ older people provides education and technical assistance for aging providers and LGBTQ organizations through its National Resource Center on LGBTQ Aging and cultural competence training through SAGECare. SAGE supports a growing network of 70 SAGECollab Partners across the country.


This position reports to the Director SAGE Center Brooklyn at Stonewall House.


The responsibilities:

  • Provide case management information referrals and advocacy to older adults; perform intakes/assessments home visits and provide case management to SAGEresidents
  • Conductbenefitandentitlementeligibilityassessmentsandassistclientswithpublic benefits applications and other entitlement programs andopportunities
  • Conduct group workshops/programs/support groups asneeded
  • Collect and enter all participant and service unit data in database(s) thoroughly and timely in accord with organizational policies and regulatorymandates
  • Assessing older adult needs and developing care planstrategies
  • Providing support and resources good relationships with external providers and developing linkages to communityresources
  • Research and provide referrals based on care plangoals
  • Assisting with benefits such as Access A Ride or Visiting Nurse services Meals on Wheels etc.
  • Responding to crisis situations and developing follow-up report with details and recommendations
  • Communicatewithfamilymembersandotherstakeholdersinvolvedintheresidents care planservices
  • Perform other duties as needed orassigned.

Need-to-haves:

  • BA or work experience in the human services field job role and/or populationserved
  • Strong organizational skills with the ability to manage and prioritize multipletasks
  • Strong written and verbal communicationskills.
  • Flexible and dependable this position requires some flexibility around scheduling; some evening or occasional weekend hours may berequired.
  • Ability to work independently as well as a member of ateam.
  • Comfortable working in a fast-paced environment.
  • Ability to organize files schedules dates and information.
  • Self-directed and able to work with light supervision.
  • Computer-literate with knowledge of Microsoft Office and ability to learn other pertinent software systems.

Nice-to-haves:

  • Familiarity with LGBT aging issues and cultural and linguistic competency regarding age ability sexuality gender identity and expression race ethnicity class and other characteristics that affect the lives of diverse older adults.
  • Familiarity with social and aging services networks and supports.

  • The union affiliation:This is a union-covered position and the successful candidate will be required to join the SAGE Workers Union and is covered by the Unions Collective Bargaining Agreement.

Monday-Friday with the occasional weekend and/or overtime
