Overview
We are seeking a dedicated and highly skilled Nurse Practitioner (NP) to join our healthcare team. The ideal candidate will have strong experience in subacute and acute care with the ability to diagnose and treat medically complex patients across multiple care environments. This position functions as a key clinical provider within a skilled nursing facility that includes both long-term care residents and a specialized ventilator/tracheostomy unit as well as performing hospital rounds in collaboration with hospitalists.
The NP provides comprehensive medical management daily rounding acute issue response and interdisciplinary care coordination to ensure high-quality patient outcomes. This role requires strong assessment skills independent clinical decision-making and the ability to manage complex patient populations in both facility-based and hospital settings.
This is a five-day flexible schedule that includes one weekend day as part of the regular weekly schedule.
Duties
Conduct comprehensive patient assessments including detailed physical examinations and medical histories.
Perform daily rounds in hospitals alongside hospitalists as well as subacute acute and skilled nursing settings.
Provide ongoing medical management within a skilled nursing facility including long-term care and ventilator/tracheostomy units.
Develop and implement individualized treatment plans based on patient conditions and clinical findings.
Perform diagnostic testing and interpret results including laboratory work.
Administer treatments including tube feeding management.
Provide education to patients and families regarding health management recovery expectations and disease prevention.
Manage chronic and complex conditions including dementia care and internal medicine-related cases.
Respond to acute clinical issues promptly and independently.
Ensure full compliance with HIPAA and organizational confidentiality standards.
Collaborate closely with multidisciplinary teams (SNF teams acute care teams hospital teams) to ensure coordinated case management and optimal utilization management.
Apply aseptic techniques in all procedures to ensure patient safety.
Maintain current knowledge of medical guidelines and best practices in advanced nursing.
Skills
The successful candidate should possess:
Proficiency in performing dialysis procedures.
Experience with dermal filler administration.
Strong laboratory and diagnostic interpretation skills.
Expertise in aseptic technique and infection prevention.
Ability to manage and oversee tube feeding protocols.
Thorough understanding of HIPAA regulations and patient privacy.
Strong case management and utilization management abilities.
Experience in dementia care and supportive cognitive management strategies.
Solid background in internal medicine for diagnosing and managing complex conditions.
