Home Health OT Occupational Therapist (PRN) Empath Home Health - South Pinellas county
Empath Home Health a part of Empath Health is growing! Were expanding our home health services in south Pinellascounty (St. Petersburg Pinellas Park Clearwater Seminole South Pasadena and surrounding areas) and looking for a passionate PRN Home Health OT Occupational Therapist to join our this PRN role youll deliver individualized care build trusted patient relationships and help people manage their health with dignity and independence.
Locations: Home health visits in south Pinellascounty (St. Petersburg Pinellas Park Clearwater Seminole South Pasadena Gulfport etc.)
Schedule: PRN; 5-10 home health visits per week.

What Youll Do
- Perform comprehensive patient assessments including OASIS assessments at required intervals
- Develop initiate and revise individualized plans of care as patient needs evolve
- Deliver skilled occupational therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless holistic care
What Youll Need
- Active Florida OT license
- Minimum of 1 year experience as a licensed Occupational Therapist (OT).
- Home health care experience strongly preferred.
- Prefers experience using Homecare Homebase (HCHB) EMR
- Strong assessment critical thinking and patient education skills
