Key responsibilities center on evidencebased assessment goaldriven treatment planning and outcomedriven progression. Youll conduct comprehensive evaluations develop PT plans of care implement therapeutic exercise programs apply manual therapy techniques and monitor progress toward functional goals. Youll lead gait and balance training assistive technology applications and functional mobility coaching ensuring safe transitions to home or community environments. Documentation in electronic health records is meticulous timely and compliant with payer guidelines. Youll collaborate with nurses physicians case managers and therapy aides to coordinate care participate in case conferences and contribute to discharge planning and postdischarge followup when applicable. Opportunities exist to specialize further share expertise through inservice trainings and mentor newer therapists as you grow within the role.
