Orientation $200 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
On Call (Daily) $3300 - Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate - $4950 - Per 24 hours to include 4 hours gratis
Call Back $400 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $600 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average worked Callback Hours: 6.62 hours
Schedule/Availability Requirements:
- Nov 27 (7a) - 29 (7a)

- 24 hour call (7a-7a)
- Total of 9-10 shifts needed per month for all locums.
- Site will not schedule providers more than 2-3 day stints.
- Setting: Inpatient Emergency Room Call Trauma Level III

- Required Procedures/Job Duties: GYN Emergencies Obstetrics including C-section.

- Dealbreakers (Do Not Present): Must be willing and able to work 24 hour shifts.
EMR Required Training:
- Credentialing Preapp and full app must be completed and submitted to move provider into applicant in training to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account.
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual (PPL) training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day.
- First Day Details - Site requires completion of EMR Modules and training before first day details can be released.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

When booking they must reference Centura Health or LUH to receive the prenegotiated rate between the facility and hotel. Providers are required to book at the preferred hotel when room are available. If rooms are not available they must follow the CommonSpirit Health travel policy.
- Air Travel: Local/drive-in candidates only - Highly Preferred
- Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation.
