Salary Range: $32 - $39/hour (dependent on education and experience)
Permanent Placement: $5000 Flat Fee
The LPN is responsible for monitoring the health and well-being of residents and provides leadership support and assistance with resident care and services.
Responsibilities include but are not limited to maintaining the overall well-being of our residents; assisting in training and monitoring medication administration; communicating with resident support services and families; and maintaining clinical quality assurance in accordance with federal state and local standards policies to promote the highest degree of quality care and services to our residents.

Essential Duties include (but are not limited to):
Identifies on-going needs and services of residents in promotion of the highest quality resident services.
Ensures proper follow through assessment and documentation for residents with a change in health care service or status.
Assists the Director of Nursing (DON) in conducting resident assessments at time of move in 30 days post move in every six months and with any change in status.
Conducts quarterly care plan updates and completes a nursing progress note to summarize any changes.
Assesses and monitors residents who have experienced a fall or other incident; documents and reports as required and directed.
Maintains frequent and timely communications with residents families and/or responsible party to ensure medical needs for the resident are being met.
Contacts residents primary care provider (PCP) when necessary and/or upon family request.
Ensures weights and vital signs are obtained monthly for each resident.
Assists in maintaining health records according to policies federal and state regulations.
Assists the Director of Nursing (DON) in readiness for state surveys.
Orders medications and equipment as needed.
Ensures resident changes concerns and/or solutions are reported to the DON as appropriate.
Regularly monitors each resident s medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accurately.
Promptly faxes new orders to the pharmacy and processes them into the MAR.
Monitors the medication room for neatness cleanliness availability of medications and expired medications.
Maintains confidentiality of all resident information.
Understands and follows infection control practices.
Reports occupational exposures to blood body fluids infectious materials sharp sticks and hazardous chemicals immediately.
Ensures oxygen tanks are stored safely exchanges residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels.
Attends regular meetings: Town Hall department team meetings care plan meetings and others as directed by the DON.

Experience and Qualifications:
Graduate of approved college / school of nursing. - Required
Maintain a current state license as a Licensed Practical Nurse. - Required
Minimum of one (1) year experience in home health assisted living hospital or long term care environment. - Required
Demonstrated knowledge of nursing practices techniques and methods.
Good assessment skills
Ability to handle multiple priorities.
Possess written and verbal skills for effective communication and level of understanding.
Competent in organizational/time management skills.
Demonstrates good judgment problem solving and decision-making skills.
Ability to work weekends evenings and flexible hours available to our customers at peak service delivery days and times.
STATE LICENSE REQUIREMENTS :
