Job Summary


Summary: St. Anthony Hospitals ENT team is looking for Urgent locum support. Coverage begins as soon as Dec 15th 2025.


BID DUE DATE: Dec 5th at 5p EST!


Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation $212.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

On Call (Daily) $3400 - Per 24 hours to include 4 hours gratis

24 Hour Holiday Call Rate - $5100 - Per 24 hours to include 4 hours gratis

Call Back $425 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Holiday Call Back $637.50 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Schedule/Availability Requirements:

  • Dec 15-17 (a)
  • Potentially 2 weekends per month ongoing pending PAC approvals for the additional coverage.

Details:

  • 24 hour call (7a-7a)
  • Setting: Inpatient Call Trauma Level 1
  • Required Procedures/Job Duties: Standard ENT coverage.
  • EMR: EPIC
  • Support Staff: None
  • Reason for Coverage: Provider resigned leaving gaps in call coverage.

EMR Required Training:

  • Credentialing Preapp and full app must be completed and submitted to move provider into applicant in training to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account.
  • Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
  • EMR Virtual (PPL) training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day.
  • First Day Details - Site requires completion of EMR Modules and training before first day details can be released.
  • All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

    • What is the name of the last facility the provider used EPIC
    • Can you provide me with a photo large enough to add to a badge and not become pixelated - Please attach photo in documents section
    • Can you disclose the providers time zone for scheduling the virtual epic training
    • We need the providers best phone # to use for call shifts while working. Can you disclose
    • We need the providers best email to use for programs while working onsite and the preapplication. Can you please send us their personal email address

Travel Preferences:

  • Preferred Lodging: Candidates must be willing to stay at one of the preferred hotels only and must request the prenegotiated rate at time of booking to be considered.
    • Sheraton Denver West Reference: Centura Health or SAH
    • Hampton Inn Reference: St. Anthony
    • Sonesta Simply Suites Reference: SAH or St. Anthony Hospital
    • Towne Place Suites Marriott Reference: SAH/St. Anthony Hospital
    • Fairfield Inn Denver West / Federal Center Reference: Centura Health or SAH.
  • Air Travel: Local/drive-in candidates - Highly preferred but not required
  • Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.


Credentialing Information:

  • Timeframe: less than a week for Temps
  • Credentialing with other facilities No
  • Requirements: Self inquiry NPDB report must be provided and CO license or Active IMLC to release preapplication.
  • Requirements: Preapp and full app must be submitted within 72 hours of receipt of initial app to allow enough time for AD account creation for EMR requirements. Once both are submitted provider is moved to applicant in training which initiates AD Account creation for provider EMR account and can take up to 5 days before release of EMR training requirements.

Requirement description :
  • Board Certified Otolaryngology - Head and Neck Surgery - REQUIRED
  • Active CO license or IMLC (LOQ in hand at time of submission - REQUIRED
  • Active BLS - REQUIRED
  • Active CO DEA - HIGHLY PREFERRED - Required for credentialing (site will consider a transferred DEA only however must keep this DEA for the duration of the assignment not just for shifts worked.)
  • Candidate must be available for any of the dates detail in job description - REQUIRED
  • Must state # of shifts per month provider can offer to the team - REQUIRED
  • Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - REQUIRED
  • Candidates must confirm they understand the preferred lodging site is a requirement when vacancies are available for booking and when not available they must follow the CommonSpirit Health travel policy - REQUIRED
  • Local/drive-in candidates only - HIGHLY PREFERRED
