Job Summary

JOB SUMMARY:

This role is responsible for the overall operation and facilitating participant transition from one level of care to the community. This will be done by addressing the preparatory needs of participants in the early stages of recovery discharge planning development of daily living skills and coping mechanisms through team and individual work in the community. The team will follow participants into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is enough to meet their needs. The role will require some on-call coverage. As a practicing clinician the Team Leader is expected to provide direct services to program participants. The position entails program oversight inclusive of staff training development and revision of systems for program operations and administration ongoing integration of program-specific policies and procedures budget planning integration of health information technology and community engagement. This role serves as a liaison with the lead contracting entity (CBC) and participates in policy and quality management workgroups.

ESSENTIAL JOB FUNCTIONS:

  • Management and oversight of the day-to-day activities of the Pathway Home team in accordance with internal and external regulatory standards and guidelines.
  • Counsel in individual or team settings with emphasis on prevention and preparing for independent community living and to promote optimum mental and emotional health. May assist participants deal with issues associated with but not limited to family and social relationships stress and symptom management activities of daily living medication management and housing readiness.
  • Participate in case conferences to help identify community health needs and tailor services to meet these needs.

  • Provide regular individual and group supervisory orienting new team members to policy and procedures as well as ongoing training and mentorship.
  • Assists with the identification development implementation and completion of quality improvement projects and plans to solve problems/issues.
  • Facilitate and monitor the organizing of staff schedules as well as leading weekly staff
  • Perform chart reviews and ensure completion of appropriate and timely documentation within Health Information Technology system.

  • Marketing and presentation of Pathway Home program to potential and historic referral sources to build up community referral base
  • Manage teams on-call duties and ensure 24/7 crisis response availability and provide crisis intervention services on a rotating basis including referrals to respite and other resources.
  • Manage recruitment of staff as well as addressing staff performance-related issues in accordance with salient policies and procedures specifically to the organization of hire.
  • Screen candidates for programming and determine Assign caseloads to team members.
  • Work with all stakeholders (members network families and government staff) in a caring and respectful manner and with due understanding of and consideration for cultural differences.

QUALIFICATIONS AND EXPERIENCE:

Masters degree in social work or Mental Health Counseling. For MSW required LCSW and for MHC required LMHC NY State Licensure. A minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field with progressively increasing responsibilities preferably with the target population.
