The Position
The Nurse Practitioner is a vital member of an interdisciplinary healthcare team contributing to the planning implementation and evaluation of individualized care plans. This role provides comprehensive primary care to older adult participants in a variety of settings including the clinic patients homes and affiliated long-term care facilities. The Nurse Practitioner collaborates closely with physicians and the broader care team to ensure the highest quality of care focusing on both acute and chronic conditions.

- Salary Range: $106000- $149000 Based on Experience

Responsibilities
- MondayFriday 8:00 AM 4:00 PM
- Evaluates participants for episodic needs acute illnesses and provides follow-up care following hospitalizations.
- Visits nursing facilities to address acute or chronic conditions when participants are unable to visit the clinic.
- Conducts home visits for participants who are homebound due to acute or chronic health concerns.
- Completes initial assessments for all new participants and conducts quarterly evaluations throughout their first year of enrollment.
- Limited Local Travel
- Oversee Care and Treatment Plans
- Collaborate as part of the Interdisciplinary Team
- Supervise and Mentor Clinical Staff
- Participate in Phone-Only Call Rotation (1 in 15 weeks)
- See 1015 patients per day
Requirements
- Licensed Nurse Practitioner in the state of Massachusetts
- Certified as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
- 1 Year of Recent Experience as a Nurse Practitioner is Required
- Mandarin Speaking Required
- Experience in primary care or geriatrics preferred
- Current DEA registration in Massachusetts
- Current CPR Certification
- COVID-19 Vaccination Required or Exemption
