Required
Degree in Occupational Therapy and registered with the Ontario College of Occupational Therapy
Three (3) years recent clinical experience.
Knowledge of and experience in the application of principles of psychosocial rehabilitation.
A reliable vehicle and valid Ontario drivers license.
Excellent organizational and interpersonal skills.
Knowledge of community resources in Hastings and Prince Edward Counties
Must be available for on call and Outreach services as required.
Must have satisfactory work performance in present position.
Must have previous proven good attendance record.
Must be capable of coping with a demanding workload (physical demands analysis in job description)
Duties
To provide client centered treatment rehabilitation and support to referred clients in Hastings and Prince Edward Counties. Conduct comprehensive client assessments.
Participate in case finding activities and engaging individuals who require ACTT service
Develop individualized client treatment/rehabilitation plans in collaboration with the other ACTT team members and the client.
Provide direct assistance with symptom management including 24 hour crisis support
Provide direct assistance with instrumental functioning employment social relations and or activities of daily living.
Completion and maintenance of all necessary documentation as per ACTT and professional standards.
Monitoring and evaluating client progress and revising treatment/rehabilitation plans as indicated.
Ensure continuity of client care is maintained through exchange of information and communication with the client family physicians and other service providers
Take a lead role in ensuring vocational care plans for ACTT patients.
Other duties as assigned.
