Job Summary

Overview

The Planning Coordinator I provides family centered coordination services to infants toddlers and their families who are eligible for the Individuals with Disabilities Education Act (IDEA)s Part C Early Intervention through Baltimore City Infants and Toddlers Program.

Responsibilities

1. Provide home- and center-based family centered services to part C-eligible infants and toddlers and their families. This includes developing collaborative
relationships with families while maintaining professional boundaries; collaborating with the family to determine the concerns priorities and needs of
the child and family; developing and revising the Individualized Family Service Plan (IFSP) as needed; and monitoring service delivery.
2. Attend and actively participate in Baltimore Infant and Toddlers Program (BITP) Part C eligibility determination evaluations and family assessments to develop the IFSP.
3. Complete and maintain monthly contacts with all families (e.g. telephone home visits agency visits). Appropriately document and submit billing in a timely manner.
4. Effectively monitor and adhere to the required timelines for BITP parts B and C. Maintain all required paperwork and enter data in a timely manner.
5. Monitor developmental milestones family outcomes and routines.
6. Complete intake referrals schedule evaluations and order assessments as needed.
7. Complete all components of the transition process including but not limited to completing transition planning with the family submitting transition packet as necessary gathering documentation for part B eligibility meetings and/or Individualized Education Program (IEP) meetings attending all related
meetings and developing an extended IFSP when necessary.
8. Serve as an integral part of the early intervention team working closely with other providers and the family.
9. Make appropriate referrals for the family and collaborate with outside agencies.
10. Provide support education and information to promote family collaboration and self-advocacy.
11. Participate in tasks to improve program quality and growth related to CFSC and the working relationship with BITP. This may include participating in mandatory program meetings and trainings planning and chairing advisory committee meetings tracking progress towards program goals maintaining involvement in community outreach and collaborating with other internal departments and outside agencies to maintain referrals and ensure best practices on behalf of the population being served.

Qualifications

EDUCATION:
Associates degree in a human-services or related field required. 2 years of experience may be substitute for this degree requirement at the discretion of
department leadership.
Bachelors degree preferred.


EXPERIENCE:
Previous work in the field of disabilities ideally with children and their families preferred.
Previous case management experience preferred.

USD $39747.76/Yr.

Maximum pay range

USD $65065.73/Yr.
