Job Summary

Ambrose Avenue Group Practice in Colchester is a friendly and welcoming GP practice and are currently seeking a respiratory nurse to join our team.Working across our two sites Ambrose Avenue and Tollgate this is a full-time role requiring 37.5 hours per week over five days. The successful applicantmust have experience with asthma and COPD.

The main responsibilities of the role include:

Undertaking health assessments administering medication and treatments and performing diagnostic tests.
Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
General health and well-being promotion as well as creating individual person-centred plans for patients with or at risk of a long term condition.
Running asthma COPD clinics and reviews.
Managing and treating long-term conditions in line with national and local policies and the practice needs.
Reviewing medication for therapeutic effectiveness in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
Prioritising health problems and intervening appropriately to assist the patient in complex urgent or emergency situations including initiation of effective emergency care.

About the Candidate

All applicants should have the following qualifications registrations skills and experience:

A relevant nursing degree.
Current NMC registration with an active PIN.
Competence to undertake asthma and COPD holding relevant certificates.
Knowledge and experience of its application in supporting long-term conditions.
Insight and understanding of current issues in nursing and the wider NHS.

Ambrose Avenue Group Practice is a friendly long-established award-winning progressive GP practice. We are based over two surgery sites within 10 minutes of each other serving the population of Stanway and Prettygate in Colchester Essex. We are a training practice and offer services to a population of over 14500 patients.The clinical team consists of eight GPs GP trainee four advanced nurse practitioners three nurses two healthcare assistants and various allied health professionals including GP assistant health and wellbeing coach clinical pharmacist pharmacy technicians care coordinator social prescriber and care advisor.

We hold monthly meetings and training sessions and have a diverse team of clinicians including first contact physio pharmacists ANP/NP care coordinator social prescriber health coach GP assistant and an award-winning team of mental health workers.
