Job Overview/Summary:
The Physical Therapist will evaluate plan and provide physical therapy services to patients in accordance with New York State regulations and professional standards. This role requires strong clinical skills the ability to work independently and a commitment to improving patient mobility and overall well-being.

Responsibilities:

Conduct patient assessments and develop individualized therapy plans.
Provide therapeutic exercises treatments and interventions.
Document patient progress and adjust care plans as needed.
Collaborate with interdisciplinary healthcare teams.
Educate patients and families on therapy processes and ongoing care.
Travel as required to patient locations.

Primary Skills:

Strong knowledge of physical therapy practices and procedures.
Ability to assess diagnose and implement therapy plans.
Excellent communication and documentation skills.
Ability to work independently and manage time effectively.

Secondary Skills (Good to Have):

Experience working in home care or community-based settings.
Familiarity with electronic charting systems.

Qualifications:

Currently registered by New York State and the American Physical Therapy Association.
Current New York State Physical Therapy license.
Current New York State drivers license and automobile insurance.
One year of experience as a therapist preferred.
