| Position Summary Under the direct supervision of the RN The Nursing Assistant provides basic patient care. Assists with data collection Performs accurate measurements and recording of vital signs and intake and output Able to care for patients in restraints Documents accurately all cares provided to include date time of entry signature and title. Demonstrates consideration of patient rights including confidentiality privacy safety patient participation in the plan of care and the right to considerate and respectful care. Answers call bells prompt Introduces self to patient Makes round every two hours to determine patient needs Understands role in the following: Code Blue Code Red (location of fire extinguisher and fire alarm) Haz Mat spills Code Pink (infant abduction) Disaster plan and evacuation route Follows the National Patient Safety Goals Uses the two patient identifiers when caring for patients Washes hands before and after patient care Follows Infection Control Policies Promptly communicates change in patient condition to team leader as appropriate Including but not limited to: Abnormal vital signs Altered level of consciousness Patient complaints of pain and distress Excessive or insufficient intake and/or output Changes in skin condition (redness skin breakdowns etc.) Patient falls Assists co-workers as needed Recieves report from RN upon arrival and provides report to the RN prior to going off duty |
