Job Title: LPN Long-Term Care (LTC)

Position Overview
A long-term care facility in Anchorage AK is seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents. This role requires strong clinical skills attention to detail and the ability to thrive in a fast-paced LTC environment.
Key Responsibilities
Provide high-quality nursing care assisting residents with ADLs mobility and routine health monitoring.
Administer medications and treatments per provider orders (wound care catheter care injections etc.).
Monitor assess and document resident conditions; promptly report changes to RN or provider.
Complete accurate EHR charting including MAR/TAR and shift documentation.
Assist in developing and implementing individualized care plans.
Perform clinical tasks such as vital signs blood glucose checks G-tube feedings (if applicable) and oxygen therapy.
Ensure resident safety through fall prevention infection control and monitoring high-risk residents.
Provide compassionate support to residents and families ensuring comfort and dignity.
Supervise and delegate tasks to CNAs ensuring proper care standards are met.
Collaborate with interdisciplinary teams (PT/OT social services physicians) to support holistic care.
Respond promptly to emergencies following facility protocols.
Maintain compliance with state regulations and LTC policies.

BLS (AHA)
