Frasier is seeking a dedicated Wellness Nurse to support residents in our Assisted Living community. This role ensures high-quality clinical care fosters strong relationships and promotes a vibrant person-centered environment where residents can thrive.
This position follows an 8:00 AM to 4:30 PM shift schedule and requires working every other weekend.

WHAT WE OFFER
- Competitive Pay: $30.00-$46.00 per hour based on experience and qualifications -plus shift differentials when applicable:
- Evening shift differential $1.75/hour
- Overnight shift differential 2.75/hour
- Weekend shift differential 1.75/hour

HERES WHAT YOULL DO
Clinical Care & Health Monitoring (40%)
- Oversee medication administration set-up and reordering per physician orders.
- Manage medical treatments and monitor residents for changes in condition.
- Assess pain and implement both pharmacological and nonpharmacological interventions.
- Communicate with physicians regarding changes test results and care needs.
- Act independently in routine and emergency situations; complete incident reports promptly.
- Serve as liaison with physicians and vendors; provide dementia-specific interventions.
Care Planning & Documentation (20%)
- Coordinate post-admission assessments and care conferences.
- Maintain accurate documentation in EMR systems; audit charts for compliance.
- Participate in quality improvement initiatives and communicate via shift reports.
Resident & Family Communication (15%)
- Communicate effectively with families and physicians.
- Respect residents cultural and spiritual preferences; deliver personalized care.
- Model Eden Alternative principles and treat residents with dignity and kindness.
Compliance & Team Leadership (25%)
- Assist with admissions transfers and discharges.
- Ensure regulatory compliance and survey readiness.
- Train and oversee Resident Assistants in medication administration.
- Support staff development through meetings and continuing education.
HERES WHAT YOULL NEED
- Current Colorado LPN or RN license.
- First Aid/CPR or BLS certification.
- 35 years of experience in assisted living or long-term care; dementia care expertise preferred.
- Strong organizational communication and critical-thinking skills.
- Proficiency in EMR systems (PCC experience a plus).
- Knowledge of HIPAA state/federal regulations and dementia interventions.
