Job Description
A reputable home health and hospice organization in Southern New Hampshire is seeking a Home Health Registered Nurse for its Merrimack this role you will provide compassionate care in a home setting for patients and families with needs ranging from basic to complex utilizing the nursing process.
Key Responsibilities:
- Work one-on-one with patients outside of a clinical setting.
- Complete initial assessments of patients to determine individual needs.
- Develop and implement an appropriate plan of care making adjustments as needed.
- Administer medication as directed.
- Provide ongoing education and support to patients and families.
- Document care provided and the patients progress toward desired outcomes.
Qualifications:
- Graduate of an accredited school of nursing.
- Active RN license.
- 1 year of acute care experience is required.
Position Details:
- Full-time permanent
- Day Shift: 8 AM-5 PM
