Position Overview

Title: Outreach Specialist (Licensed Therapist)
Department: Treatment Services
Reports to: Clinical Supervisor

Essential Job Functions:

  • Track and Monitor all Referral and Intakes
  • Adhere to agency standards of required documentation including assessments consents screening tools progress notes and any additional intake documentation
  • Participate in staff meetings case conferences utilization review staff development clinical supervision and trainings
  • Assess all new admissions for Medicaid eligibility while maintaining compliance with billing targets and Medicaid documentation
  • Identification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skills.
  • Liaison to community based services and groups including civic social service government and business organizations.
  • Other duties as assigned by supervisor
  • Travel may be required.

Intake experience is crucial for this role.

Job Requirements

Educational Requirements:

  • Clinical Licensure Required-LMSWLMHCLCATLCSW
  • Working knowledge of various computer programs including e-mail Microsoft Word Excel PowerPoint and Windows Operating System.

Experience Requirements:

  • Two years experience with individuals with mental illness substance abuse issues and/or a history of homelessness.
  • Bilingual English/Spanish a plus.
  • The ideal candidate will be creative innovative open to change and willing to contribute new ideas.
