Position Overview
Title: Outreach Specialist (Licensed Therapist)
Department: Treatment Services
Reports to: Clinical Supervisor
Essential Job Functions:
- Track and Monitor all Referral and Intakes
- Adhere to agency standards of required documentation including assessments consents screening tools progress notes and any additional intake documentation
- Participate in staff meetings case conferences utilization review staff development clinical supervision and trainings
- Assess all new admissions for Medicaid eligibility while maintaining compliance with billing targets and Medicaid documentation
- Identification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skills.
- Liaison to community based services and groups including civic social service government and business organizations.
- Other duties as assigned by supervisor
- Travel may be required.
Intake experience is crucial for this role.

Job Requirements
Educational Requirements:
- Clinical Licensure Required-LMSWLMHCLCATLCSW
- Working knowledge of various computer programs including e-mail Microsoft Word Excel PowerPoint and Windows Operating System.
Experience Requirements:
- Two years experience with individuals with mental illness substance abuse issues and/or a history of homelessness.
- Bilingual English/Spanish a plus.
- The ideal candidate will be creative innovative open to change and willing to contribute new ideas.
